Federal Forms

  • FAFSA on the Web Application
    Students can complete the Free Application for Federal Student Aid on the Web (FOTW) to apply for federal student aid and/or the HOPE or Zell Miller Scholarship. See Apply for Aid for more information.

HOPE Forms

  • HOPE Scholarship Transient Request
    This request must be completed if you are a UGA student enrolling in classes at another HOPE Scholarship eligible school by signing in to the MyOSFA Portal.  You may wish to print a copy of the request for your records. Remember to keep track of the dates that your academic transcript must be received by UGA Undergraduate Admissions. Failure to comply may result in cancellation/repayment.   • Certification of Non Enrollment
    You must complete this certification by signing in to the MyOSFA Portal if you are a UGA student who completed a HOPE Transient Request to enroll in classes at another HOPE eligible school and subsequently did not attend the school. You may wish to print a copy for your records.  Failure to comply may result in cancellation/repayment. OSFA Forms

  • Federal Direct Parent PLUS Loan Request Form
    This form must be completed by parent borrowers requesting the Federal Direct Parent PLUS loan. Please complete the form and return it to the Office of Student Financial Aid. Be sure to follow all steps. Incomplete forms cannot be processed.
  • Computer Expense
    Please complete, sign, and return to OSFA if you wish your Cost of Attendance (COA) to be adjusted to reflect the expense of a computer purchase. If you have questions, please call (706) 542-6147, and ask to speak to a Loan Counselor.
  • Dependent Care Expenses Request
    Please complete, sign, and return to OSFA if you wish your Cost of Attendance (COA) to be adjusted to reflect dependent care expenses you are incurring in order to attend college. If you have questions, please call (706) 542-6147, and ask to speak to a Loan Counselor.
  • Scholarship Reporting
    Sign in to the MyOSFA Portal to report to OSFA any outside scholarships and sources of assistance you will receive from sources other than UGA OSFA.
  • Private Education Loan Applicant Self-Certification
    If a student applies for a private education loan, the student must also submit this form to the lender.  Contact our office for assistance in completing Section 2: Cost of Attendance and Estimated Financial Assistance.
  • Satisfactory Academic Progress (SAP) Appeal
    If you have been notified that you are not making Satisfactory Academic Progress (SAP) for Federal Student Aid (FSA) and State of Georgia Financial Aid eligibility purposes and wish to submit an appeal for consideration, you can complete, sign, attach the required documentation and send to OSFA. You may wish to make a copy for your records. Appeals are reviewed in the order in which they are received. If you have questions, please call (706) 542-6147, and ask to speak to a SAP counselor.
  • Scholarship Certification Statement
    In order for UGA OSFA to complete your Robert C. Byrd, Georgia HERO, Law Enforcement Dependent Grant, or Georgia Public Safety Memorial Grant application, this form must be completed, signed and returned. OSFA cannot process your application until this statement is received. If you have questions, please call (706) 542-6147, and ask to speak to a scholarship counselor.
  • Study Abroad Expense Request Form
    This form is for use by students studying abroad who would like to document the additional costs associated with such programs. Students should contact OSFA to determine if such additional costs would result in increased eligibility before submitting this form. If you have questions, please call (706) 542-6147, and ask to speak to a counselor.
  • Less Than Full-time Exception for Academic Scholarship
    Academic Scholarships including the Alumni Associate Endowed, Baldwin, Charter, Drew Charter, Georgia Incentive, Glenn Family, Goizueta, Harris, Henry King Stanford, Hoard Memorial First Tee, One UGA, Presidential, Rothberg/KIPP, Woodruff, and the Classic Scholarship require full-time enrollment each semester. Each scholar can receive a one-time less than full-time exception during a semester of enrollment. This form is for use by scholars (of the scholarships listed above) to receive the scholarship for less than full-time enrollment during one of their semesters of eligibility.   • Change in Aid Request
    Complete the Change in Aid Request by signing in to the MyOSFA Portal. If you have questions, please call (706) 542-6147, and ask to speak to a Counselor
